Terms and Conditions AgreementThe Cazes Care Membership is an exclusive membership with Cazes Family Dentistry, LLC and Sparta Dental Associates, LLC. It is not a traditional dental insurance, but a smart insurance alternative. It is available to everyone who applies. Memberships cannot be transferred to an outside dental office. Accounts must be active and in good standing to receive membership benefits. It is the patient's responsibility to schedule and keep appointments. There are no refunds of annual service fees for any reason, including failure to schedule or maintain appointments. You may cancel the plan at any time.
